Four-Step Evaluation Model

From intake to long-term review.

  1. Intake and assessment establish baseline needs and goals.
  2. Service tracking documents assistance, referrals, case notes, receipts, and follow-up.
  3. Progress review occurs at 30 days, 90 days, six months, and 12 months when practical.
  4. Leadership and the Board review programs quarterly.
